The Fourth Annual Family Caregiver Summit

The Ninth Annual Family Caregiver Appreciation Luncheon

  • When: Monday, November 17th, 2008
    Where: Bartow Civic Center.

 

 

What we are up to now:
We just were approved for a Polk HealthCare Plan Community Outreach Grant for a project involving caregiver aid and training to the estimated 1,500 indigent Alzheimer’s family caregivers who can be classified as “medically underserved” according to the FL Dept. of Elder Affairs.

The focus of this program will be to:

  • Decrease health disparities by providing these caregivers with the information and the resources they need to facilitate their family caregiver activities.
  • Promote healthy lifestyles and environments by showing these indigent family caregivers that it is important to take care of themselves so that they can take care of their loved one
  • Improve access to health care by providing the resource information that is typically not available to, or known by, many family caregivers

 

Family caregivers already enrolled in the Polk HealthCare Plan will already be eligible. The others will have to follow the rules and regulations set forth in the Polk County Indigent Health Care Oridnance No. 03-89

PCFC will coordinate this entire program. Qualified trainers will provide the educational and informational sessions, and we plan to use personnel and facilities that are already available in Polk County. We desire to have classroom settings and “hands on” educational opportunities for these indigent family caregivers.

The desired outcome of this program will be to ensure that there is a higher quality of care for care –recipients, and it will help to prevent institutional placement of care-recipients as long as possible.Caregivers will have better training and be better equipped in their role as a caregiver. This should lower costs for the caregiver, the care-recipient, and the community at-large.
